12. For ALL Business Types: Are all changes to vendor/client/customer contact and/or bank account details agreed in writing, confirmed and validated over the telephone with the client/customer?

13.a. Are all names, addresses, sort codes, account names and bank account numbers checked by the administrator / account manager at the Insured?

13.b. Is a written / digital contemporaneous record kept confirming the details held on the Insured’s internal records and validated by the client are the same as those being used?

13.c. After the payment form has been completed, signed and dated by the administrator / account manager, is it then double checked, signed and dated by a supervisor before transferring funds?
